To be submitted if your club has marked a referee 50 or less than 50.
Please refer to the League handbook, where there is a Guide to Marking Referees, before you award the mark you feel appropriate after every match.
Rule 13g requires that you submit a report for a mark of 50 or less than 50, and this form gives you the means to submit such a report. This report should be sent with the result sheet to the fixtures secretary, who will in turn forward this to the referees appointments officer, along with the referees marks. The report is intended to be used constructively in an effort to improve standards.
